Equipping you to understand more about personalised eye care and its potential within your community, and engage with policy makers to support greater use of personalised care
The Personalised Eye Care Toolkit was created on the basis of the Personalised Healthcare Hub and Toolkit for the Patient Community and was co-created with patient community leaders. It includes a suite of educational resources on the topic of personalised eye care and its benefits for people and communities affected by retinal diseases.
The toolkit is for patient advocacy groups, patient community leaders and people living with low vision and their supporters. The resources have been created in a variety of formats so you can use them in different situations, such as meeting and event presentations, handouts and as conversation starters. The toolkit has been adapted to people living with low vision.
To increase understanding of personalised healthcare and its potential to benefit people affected by health conditions, you can share the Personalised Eye Care Toolkit, the Personalised Healthcare Toolkit for the Patient Community, and the Personalised Healthcare Hub, with members of your community. You can find guidance on how to share here.
